How much tooth do you actually have?

The more healthy tooth structure you have left, the more options you usually have.

A patient looks at an old filling in the mirror and says, "It doesn't hurt. Is it really a problem?" The tooth feels normal. It chews fine. Nothing looks broken from the outside.

But dentistry is not only about how a tooth feels today. It is also about how much tooth is left to support the next decision.

In the Keep Your Teeth Framework, this chapter is about Structure. Structure means the amount and quality of natural tooth remaining. It includes the walls of the tooth, old fillings, cracks, root canal history, and how much reserve the tooth still has.

Structure is the tooth's foundation

A tooth is not just a hard white object. It is a small structure built to handle chewing forces every day.

When a tooth is healthy and mostly intact, it has strong walls. Those walls support each other. The tooth can flex slightly under bite pressure and still hold up.

When a tooth has a small filling, much of that structure is still present. The filling repairs the damaged area, but the tooth is still doing most of the work.

When a tooth has a very large filling, the situation changes. The filling may look fine, but the tooth may have less reserve. The remaining walls may be thinner. The tooth may still chew normally, but it has less margin for error.

This is why structure matters. It sets the ceiling for what the tooth can tolerate.

A tooth can look fine and still have low reserve

Patients often think a tooth is either broken or not broken. In reality, there is a middle zone.

A tooth can look stable today but have reduced long-term tooth stability. This often happens with large old fillings. The filling may still be sealed. There may be no pain. The X-ray may not show anything dramatic.

But the tooth may have lost a lot of its original design.

Think of a wooden chair. A new chair has four strong legs and a solid frame. If one part breaks and is repaired, the chair may still work. If several parts have been patched over the years, the chair may still look usable. But it no longer has the same reserve.

You would not judge that chair only by whether it holds you once. You would ask how it will hold up after years of daily use.

Teeth work the same way.

Large fillings change how force moves through the tooth

A filling replaces missing tooth structure. But it does not always restore the tooth to its original strength.

This is especially important in molars. Molars carry heavy chewing pressure. When a molar has a large filling, the remaining walls can act like thin wooden boards around a repair. They may hold for years. Then one day, under normal chewing, a wall cracks.

That crack may not happen because the patient did something wrong. It may happen because the tooth had been losing reserve for a long time.

This is where the dental threshold becomes important. The threshold is the point where watching the tooth may no longer preserve it. A tooth below threshold may be safe to monitor. A tooth at threshold may need treatment to protect what remains.

Structure is one of the main reasons dentists discuss when to get a crown. A crown is not automatically better. It is also not automatically too aggressive. The real question is whether the remaining tooth structure can still support a filling under normal bite pressure.

Thin walls are the warning sign

One of the most important structural signs is thin remaining tooth walls.

A tooth with thick walls can often handle a filling well. A tooth with thin walls is different. Thin walls can bend, flex, or crack under chewing pressure. This is especially true when the filling runs across the middle of the tooth or replaces key ridges.

Marginal ridges are the raised edges on the sides of back teeth. They help hold the tooth together. When those ridges are lost, the tooth may become weaker even if the filling looks smooth and clean.

This is why "just make the filling bigger" is not always the most conservative choice.

Sometimes a bigger filling saves tooth structure. Sometimes it creates the next fracture. The difference depends on how much structure is left.

Irreversible treatment should have a reason

A crown requires removing additional tooth structure. That makes it an irreversible dental treatment. Once a tooth is shaped for a crown, it cannot go back to being an untouched tooth.

That does not make crowns bad. Crowns can be protective when a tooth has already lost too much structure. They can hold thin walls together and reduce fracture risk. For the right tooth, at the right time, a crown can help preserve the tooth longer.

But a crown should not be done just because a filling is old. It should not be done only because a tooth has a history. The reason should be structural.

The better question is: "How much natural tooth is left, and is it enough to hold up?"

That question leads to better dental decisions.
