Sirolimus is an mTOR inhibitor used after organ transplant to prevent rejection and increasingly in some cancers and rare diseases.

Sirolimus is one of the most common medication causes of recurrent aphthous-like mouth ulcers. Many transplant patients on sirolimus develop persistent mouth sores that significantly affect eating and quality of life. The ulcers respond to dose adjustment and topical care but rarely resolve fully while on the medication. Coordination with the transplant team is part of management.
